I have thought some more about this and I am so impressed you posted this. If I don't overstep, I would like to recommend an approach created by a school-oriented group that now consults on several organisational levels, Culture of Dignity (https://www.culturesofdignity.com/meet-the-team). They had an example where they ran a schoolwide survey fairly regularly looking at the ten elements of dignity and how well the organization as a whole was addressing these in daily life. This allowed them to build a culture where you didn't have to wait for the exit interview to intervene in areas of tension. They instituted a whole program where they educated and lived the ten elements and constantly adjusted for this so the humans in the org (the school) all adjusted as well and learned to handle inter and intrapersonal relationships (across all levels) far more effectively. Really useful in any org, I think.
